Athletic Forms & Documents
The following form must be completed in order for student athletes to be registered as a member of any team at Cristo Rey Jesuit High School. Please note, you will need a parent/legal guardian to complete or submit these forms.
Each student athlete must submit a current, valid physical to the athletic office each year. Athletic physicals are valid for 365 days/1 year after completion. It is best practice for student athletes to obtain and submit a physical each summer before the school year begins.

HOW TO REGISTER
STEP 1: The Student Information AND Emergency Contact information must be completed for EACH sport season your son/daughter participates in. This information is required for student athletes to begin training and practicing with their chosen team.
STEP 2: Please include any insurance information if available.
STEP 3: Accept and Acknowledge CR HANDBOOK POLICIES
STEP 4: The IHSA CONCUSSION CONSENT must be read and the acknowledgement agreed to and e-signed by the parent/legal guardian of each student athlete ONCE EACH SCHOOL YEAR.
STEP 5: SPORTS PHYSICAL
Please check all of the following before uploading a sports physical.
- Are all questions answered?
- Is the student signature on the page
- Is the parent signature on the page
- Did the physician mark “yes, no or restrictions”?
- Did the physician sign the document?
- Did the physician date the document?
If all items are complete, you may upload your sports physical.
